The CMS GUIDE Model (Guiding an Improved Dementia Experience) is a transformative shift in how Medicare supports dementia care. GentleNext provides the infrastructure to execute this model—caseload management, assessment scoring, care coordination, and family support—with a focus on simplicity and HIPAA compliance.

Built for the 2am Test

Dementia caregiving is hard, often hardest at 2:00 AM. Our platform is designed to be "calm, clear, and companionable"—not another digital distraction. We prioritize human connection between navigators and families, using AI only as a silent assistant to reduce the documentation burden on care teams.
